Tasks & Duties
• Provides advice on medicines to individual patients, particularly to those who require complex drug therapy
• Liaises with physicians, nurses and other fellow health care professionals to ensure the delivery of safe, effective and economic drug treatment
• Gives consultations and recommendations to medical and nursing staff concerning drug information inquiries such as dosage and storage of pharmaceuticals, pharmacokinetic monitoring, drug interactions, incompatibilities, reporting patient side-effects and medications errors
• Conducts and evaluates medication histories, assessing compliance and suggesting modifications so as to achieve desired outcomes; instructing patients in the proper use of prescribed drugs; participate in patient care rounds with multidisciplinary teams to evaluate patient progress
• Coordinates and monitors parenteral and enteral feeding formulas
• Provides in service education program for clinical staff: e.g. clinical cases presentation, journal clubs, medical lectures/conferences, guidelines and protocols on the appropriate use of medications
• Patient Assessment/ Gathering relevant information about the patient
• Read the patient chart
• Review the patient profile
• Prepare medications history / reconciliation with home medications
• Review Lab tests: creatinine clearance, potassium, sodium, INR, CPK .
• Evaluation of medication therapy/Systematic analysis of medications prescribed during hospital stay
• Analyze and follow up on physician order to identify drug related problems such as appropriate dose, dosage form, administration time, potential drug-drug/food-drug interactions, side effects, regular follow up on specific lab values
• Optimize drug prescribing practices in terms of safety, efficacy and cost by communicating the relevant interventions to healthcare providers through active participation in multidisciplinary rounds
• Documentation of all significant patient care recommendations, intervention in the patient medical chart
• Follow up evaluation and medication monitoring during patient hospital stay and at discharge
Job Qualifications and Requirements
• Education: Pharm D or equivalent accredited residency program
• License from the Lebanese ministry of public health for pharmacy practice
• Being affiliated the Lebanese order of pharmacist
• Minimum 3 years experience as a hospital/clinical pharmacist in clinical academic setting of which 1year clinical pharmacy advanced internship in hospital setting
• Proficiency in spoken and written English and Arabic
• Good command of computer system
