Coordinate the integrated services across the Ministry of Education, Ministry of Health and Ministry of Community Development to improve outcomes for children.
Develop and lead the referral, registration and case-management system and processes.
Ensure a coordinated response to families in receipt of integrated services support.
Actively seek innovative, evidence-based and creative approaches to improve early childhood outcomes.
Develop and support the promotion of various initiatives which form part of the community center s integrated services.
Plan and deliver services in a manner that ensures the safe, smooth and efficient functioning of the center.
Develop and maintain effective and collaborative professional working relationships across government entities which improve outcomes for children and families.
Conduct meetings with families to assess needs and recommend appropriate services.
Maintain case files by recording relevant information using the integrated services database to capture information from all 3 government entities in one place.
Provide individualized support and advocacy to families.
Host multi-agency meetings to ensure identified needs are being met.
Establish and manage protocols for the shared use of rooms within the center.
Sign post families to other appropriate services.
Develop and support the promotion of various initiatives which form part of the community center s integrated services.
Promote the services of the Center in the local area.
Analyze data to ensure resources are being targeted to priority areas.
Ensure confidentiality of information is preserved at all times.
Record data to monitor, evaluate and report on the effectiveness of the center at meeting the needs of local families.
Identify and take action to address gaps in provision or to improve performance.
Assist in developing evaluations of the impact of services on improving outcomes for children and families.
Practice in a manner that ensures compliance with all Inspection and Evaluation requirements.
Identify and share examples of good and innovative practice in order to further develop the scope of services available.
Work in a manner that addresses key strategic priorities.

ملخص المرشح المطلوب


Bachelors degree in relevant field (Social Sciences, Community Development, Education, Early Childhood) with GPA 3 and above
IELTS 6 Academic or 6.5 General for non-native English speakers
2years of experience working in an integrated service setting.
Experience delivering family support, health promotion or parent education programmes.
Evidence of recent professional development
